2022 General Local Election - Mail Ballot Application Form

Mail ballot voting is now open to all qualified resident and non-resident property electors of Port Moody. Any person who meets the qualifications to vote as either a resident or non-resident property elector in Port Moody can apply to vote by mail. 

Applicants can fill out the online form below or download a PDF that can be submitted in-person at the Legislative Services counter at Port Moody City Hall (100 Newport Drive) or mailed to the Elections Office at 100 Newport Drive, Port Moody B.C. V3H 5C3.

Valid identification that provides evidence of your identity and place of residence is required in order to apply for a mail ballot. You must upload a copy of valid identification below that provides evidence of your identity and place of residence. This can be one ID (such as a BC Driver’s Licence), or a combination of IDs, such as a Canadian passport and a tax or utility bill. 

Examples of acceptable forms of IDs, for the purposes of applying for a mail ballot, include

  • BC Driver’s Licence
  • BC Services Card
  • BCID Card
  • Owner’s Certificate of Insurance and Vehicle Licence issued by ICBC
  • Social Insurance Number Card
  • Citizenship Card or Certificate
  • Birth Certificate
  • Canadian Passport
  • Property tax notice
  • Utility bill
  • Debit or credit card or statement issued by a financial institution
  • Student ID card
  • Government-issue documents such as cheque stub, Pension Plan benefits statement, Old Age Security statement
  • Income Tax Assessment Notice
  • Canadian Forces Identification

 4. Declaration and Agreement 

I understand and acknowledge that: 

  • it is an election offence:
    • to interfere with another person’s right to vote;
    • to provide false or misleading information or to make false or misleading declarations;
    • to obtain a ballot by fraudulent or improper means; and that
  • I, as an elector for the City of Port Moody, am solely responsible for ensuring that my mail ballot is received by the Chief Election Officer (or designate) no later than 8pm on October 15, 2022 to be counted in the 2022 General Local Election.
